Purpose
The purpose of this memo is to provide information to the Franklin Board of Mayor and Aldermen (BOMA) concerning the purchase of three additional Rad-57 Pulse CO-oximeter units. These units have the technology to measure carboxyhemoglobin, methemoglobin, oxygen saturation, and pulse rates on patients treated by the Franklin Fire Department and fire suppression personnel engaged in firefighting or other hazardous materials type operations where these exposures may occur.  This purchase is for new equipment for use in the Franklin Fire Department.
 
 
Background
According to the CDC, each year, more than 400 Americans die from unintentional CO poisoning, more than 20,000 visit the emergency room and more than 4,000 are hospitalized due to CO poisoning. Fatality is highest among Americans 65 and older. Firefighters are at very high risk for exposure to Carbon Monoxide in the overhaul phase of fire operations and the effects of this exposure can cause both immediate and long term health concerns.  The implementation of CO screening in the rehabilitation of firefighters is a critical component in identifying and helping to prevent the effects of these exposures.  
 
In 2011, the Fire Department purchased six (6) of these handheld units to place on the fire apparatus with advanced life support capabilities.  In 2013, the department increased the number of advanced life support apparatus from six to nine in the City.  The purchase of these additional three units is intended to add this monitoring capability to these additional three fire apparatus.  The Rad-57 monitor is the only technology currently available to non-invasively monitor for CO exposure, thus allowing this measurement to be obtained in a pre-hospital setting.
 
Financial Impact
The Masimo Corporation is the sole-source vendor of this technology and has indicated that in writing.  They are offering discounted pricing based on the quantity listed in the quote.  A sole-source letter is attached.
 
There is $20,000 allocated for this purchase in the fire department FY15 budget.  The quoted pricing is $18,300 including shipping.  The vendor has indicated in writing that the quoted unit pricing is the best municipal pricing available at this time and in the foreseeable future for the quantity indicated and staff believes the unit pricing to be reasonable for this industry at this time.
 
Recommendation

Staff recommends that the City accept the pricing quoted by Massimo Corporation and approve the sole-source purchase for the items mentioned in the amount of $18,300.  Purchasing Manager Brian Wilcox is of the opinion that this proposed sole-source purchase appears to be justified and the fire department's recommendation appears to be made in a fair and impartial manner.
