Purpose
The purpose of this memorandum is to provide information to the Franklin Board of Mayor and Aldermen (BOMA) concerning Amendment No. 5 to the Professional Services Agreement (PSA) (COF Contract No. 2013-0034) with CDM Smith, Inc.

Background
A brief history of the PSA (COF Contract No. 2013-0034) with CDM Smith, Inc. is as follows:

* April 23, 2013 COF Contract No. 2013-0034, at the request of TDOT,
which allowed for the redesign of the project to included traditional signalized intersections.

* November 24, 2014 Amendment No. 1, at the request of TDOT, which allowed for the
redesign of the project as a two (2) lane facility.

* February 9, 2016 Amendment No. 2, at the request of TDOT, allowed for right-of-way
(ROW) plans revisions.

* April 11, 2017 Amendment No. 3, at the request of TDOT, allows for additional ROW
Plans revisions, revisions to the Erosion Prevention and Sediment Control Plan, and additional geotechnical work.

* September 12, 2017 Amendment No. 4, at the request of TDOT, allows for Plan revisions to
prepare for bidding

Amendment No. 5 provides additional work that is necessary to update the plans to allow for bidding and ROW revisions. The full details of this additional work can be seen in Attachment A.

Financial Impact
The cost of Amendment No. 5 is $32,640.00. TDOT will reimburse this additional cost as a part of the project. City staff will work w...
